Recently, the family found a mass on the thyroid gland of the elderly, so they made an appointment for on-site thyroid ultrasound service through the Internet hospital system of Zhongshan Hospital affiliated to Dalian University.

After the hospital received the patient's appointment, the smart platform immediately matched Dr. Zhang Zhaoqing of the ultrasound department for on-site service. At 1: 30 pm that day, she put on her portable monitoring equipment and set off from the hospital with a portable ultrasound machine. According to the director of the medical development department of the hospital, the household service targets are the elderly or disabled elderly, patients in convalescence and end-stage patients and other people with mobility problems. To this end, the hospital has mobilized a team composed of general practitioners, ultrasound doctors, health managers, psychological counselors, public health doctors and specialist (practicing) nurses. The out-of-hospital nursing team consists of 20 head nurses and 100 specialist nurses, which are divided into wound pressure ulcer group, pipeline maintenance group, static therapy group and health guidance group.

Citizens can make an appointment service through telephone, small program, WeChat official account, Internet hospital and other ways.

In 2024, the medical care home service will be fully upgraded, and more than 70 door-to-door service projects such as home care, western medicine home care, and traditional Chinese medicine home care can be carried out. The service scope covers five districts in Dalian, with more than 3,000 registered patients. The average annual home service is about 1,000 times. It not only reflects the convenience and efficiency of smart medical services, but also meets the strong demand of ordinary people for high-quality medical services.

Two-way referral, from grassroots to "top three", hospitalization in one step

Graded diagnosis and treatment is the highlight of the crucial stage of the new medical reform. Nowadays, many elderly people have the habit of "solving minor illnesses in the community", but once they get a serious illness, can patients be allowed to reduce their procedures and admit to a big hospital as soon as possible.

Recently, Aunt Li, 71, who lives in Nanshan Community, Airport Street, was successfully and quickly referred from Nanshan Community Health Service Station to the general practice ward of the Central Hospital through the general practice software platform of the Affiliated Central Hospital of Dalian University of Technology (Dalian Central Hospital).

Without going to the hospital to register in person, she went through the hospitalization formalities directly in the ward and was admitted to the general practice department. Five days after hospitalization, the symptoms of redness, swelling, heat and pain in Aunt Li's left calf were completely relieved. Doctors sort out relevant medical information and transfer it to general practitioners of Nanshan Community Health Service Station through the general practice platform software, which will continue to be used for follow-up diagnosis and treatment.

The director of the Department of General Practice of the Central Hospital said that the hospital took the lead in introducing general practice platform software in Liaoning Province. On this platform, patients can complete two-way referral, make appointments for diagnosis and treatment, and follow-up reminders.

Patients can complete daily laboratory tests and examinations in the community, and enjoy the "family doctor" service of general practitioners in top three hospitals. The platform also establishes a complete health record for patients, and keeps each examination record, which is convenient for doctors to manage chronic diseases and provide professional guidance.

At present, patients can complete the contract of general practice platform software through two ways:

1. It is in the outpatient clinic and ward of general practice department

2. It is a medical consortium unit signed through the platform

In the future, the Central Hospital plans to expand this service to more medical consortium units, further improve the coverage and quality of medical services, and bring convenience to more patients.

Citizens only need to use Liaoshitong "e Dalian" WeChat applet, and after selecting the specific hospital to see a doctor, the system will automatically link to the WeChat official account of the corresponding hospital to complete the registration operation, providing the general public with one-stop services of pre-diagnosis appointment, in-diagnosis settlement and post-diagnosis report inquiry.

There are five pilot tertiary public hospitals, including the First Affiliated Hospital of Dalian Medical University, the Second Affiliated Hospital of Dalian Medical University, Zhongshan Hospital affiliated to Dalian University, the Central Hospital affiliated to Dalian University of Technology (Dalian Central Hospital) and the Second People's Hospital of Dalian.
